Our Family Services staff is hosting an Application Blitz on Tuesday, November 26th from 1 to 3 pm. See us at 1195 W 25th St in Jacksonville.

The following items will be needed to complete your application:

• Recent picture of you and your family
• Valid form of ID
• Social Security Card
• 30 days of pay stubs (most recent)
• Benefits letters (if applicable)
• W2s for last 2 years
• Bank statements (2 most recent)
• Completed tax returns (2 most recent)
